Health Related Social Needs
HRSN stands for Health-Related Social Needs.
These services are non-medical supports provided
through state Medicaid programs (like the Oregon Health Plan)
to help people with living and economic challenges
—such as unstable housing, lack of food, or extreme weather vulnerability—
that directly impact their overall health

Rental and utility assistance for up to six months (not designed for immediate emergency evictions)

Home modifications like wheelchair ramps, safety rails, or air-filtration and pest remediation

Tenancy and moving support services, plus storage fee coverage

Are you planning to apply for HRSN housing benefits?
Not all OHP members will qualify for HRSN housing benefits. Due to federal requirements, to qualify for HRSN housing benefits you must have specific health, living, and financial conditions. Read this overview to see if you might qualify.

HRSN housing benefits aren’t meant for emergencies. It takes time to approve and receive HRSN housing benefits. Times can vary by service and provider. If you’re facing imminent eviction, you should contact your regional Community Action Agency (CAA).

HRSN rent and utility assistance is a one-time benefit per household. If you or someone in your household has already received this benefit, you are not eligible for additional rent and utility assistance.
Who Will Qualify for These Services?
To qualify, OHP members must be in at least one of these life transitions. Additional criteria also applies for each type of HRSN service.

Release from incarceration in the past 12 months

Discharge from an Institution for Mental Disease (IMD) in the past 12 months

Current or past involvement in the Oregon child welfare system

Transitioning from Medicaid-only to dual eligibility (Medicaid and Medicare) status within the next three months or has transitioned in the past nine months

Being homeless or at risk of becoming homeless.
